So 26-Jan has come around again!


This year sees me weigh in at 66!


After first presenting with the symptoms in November 1998 and then having a radical nephrectomy (kidney removal) due to cancer in September 2001 I guess I didn’t expect to get this far!


After MRSA, numerous minor ops for bladder cancer New Years eve 2010 saw me on an operating table again – A ‘students dream New Year’s Eve‘ – Flat on my back surrounded by nurses and high on drugs!


It was fairly obvious the disease had moved on and eventually after the shambolic incompetence of The NHS under Welsh management, more interested in vanity projects and political showboating, at the end of March I finally received the diagnosis of tumours in the abdomen and lymph system!!


Yes due to the utter incompetence of the NHS in Wales it had taken 3 months to organise an urgent scan in a known case of metastatic cancer and provide a result!!


Prognosis was about 3 months!


So I opted for high dosage Chemotherapy followed by about a month of daily dosage Radio Therapy (Radiation Treatment!).


Well a marriage, a home move and much more I would ESPECIALLY like to thank Dr. Lester and his team – without whom I would NOT be here. Gerald and Carol for their incredible support and of course Lee who after 26 years together I have finally married!


When you consider that when I was born in 1946 Britain and her allies had just won a World War and there were just under 2 Billion people on the planet.


2012 has arrived and we seem to have lost a war and we are now governed with absolutely no meaningful pretence of democracy by our Continental enemies and have all but abandoned our allies, we have still found ourselves heavily in debt but this time for rather less honourable reasons as a result of economic illiteracy of our own utterly incompetent and seeminly malign Labour Government and with very little light at the end of the tunnel – not least of all due to a rise in world population just before Christmas to over 7 Billion.


That is not all that has changed as you will note from when I was 9, 2 years before I flew to Singapore alone and found myself embroilled in The Suez crisis and stranded in Ankara!:

I’ll tell you one thing, if things keep going the way they are, it’s going to be impossible to buy a week’s groceries for $10.00.

Have you seen the new cars
coming out next year? It won’t be long before $2,000.00 will only buy a used one.

Did you hear the post office is
thinking about charging 4 cents just to mail a letter.

If they raise the minimum wage
to $1.00, nobody will be able to hire outside help at the store.

When I first started driving, who
would have thought gas would someday cost 25 cents a gallon. Guess we’d be better off leaving
the car in the garage.



I’m afraid to send my kids to the
movies any more. Ever since they let Clark Gable get by with saying DAMN in GONE WITH THE WIND, it seems every new movie has either HELL or DAMN in it.



I read the other day where some
scientist thinks it’s possible to put a man on the moon by the end of the century. They even have some fellows they call astronauts preparing for it down in Texas .


Did you see where some baseball
player just signed a contract for $50,000 a year just to play ball? It wouldn’t surprise me if someday they’ll be making more than the President.

I never thought I’d see the day
all our kitchen appliances would be electric. They’re even making electric typewriters now.

It’s too bad things are so tough
nowadays. I see where a few married women are having to work to make ends meet.

It won’t be long before young
couples are going to have to hire someone to watch their kids so they can both work.

I’m afraid the Volkswagen car
is going to open the door to a whole lot of foreign business.

Thank goodness I won’t live to see the day
when the Government takes half our income in taxes. I sometimes wonder if we are
electing the best people to government.

The fast food restaurant is
convenient for a quick meal, but I seriously doubt they will ever catch on.

There is no sense going on short
trips anymore for a weekend. It costs nearly $2.00 a night to stay in a hotel.

No one can afford to be sick
anymore. At $15.00 a day in the hospital, it’s too rich for my blood.



If they think I’ll pay 30 cents for a haircut, forget it.

Hi,

This was my first full CT Scan since the joys of Chemo & Radio Therapy earlier in the year!

Scans come with their own mix of anticipation and dread! I dubbed it ‘Scanziety’ some 12 or 13 years ago when I first started down this cancer route!

Of course you want the scan but naturally one fears bad news and there is a constant feeling that perhaps ignorance is bliss – though to be honest I find underastanding EXACTLY where I stand and the depth of the ordure gives me an understanding of just how grim is the battle ahead at this stage and thus I ‘feel’ I have a greater ability to deal with it!

Although I was due at 14:30hrs. at Velindre for my appointment I arrived at 13:35hrs. as I thought I might be down for ‘bloods’ as well and hoped to do that first as there is quite a bit going on at home. Seems they don’t want my blood this time! So I sauntered around to Radiology to check in before going for a coffee in the canteen.

Good news ‘can I do you now sir?’ and I was in and changed into one of those idiotic gowns in minutes – drink another pint of water quick stab to put a cannella in and then a top up of iodine to improve disclosure on the scan.

10 minutes later and I’m free to go and it isn’t quite 14:00hrs!

Now for ‘Super Scanziety as we wait for the results on Friday!

Hi,

well a whole week since my last Radio Therapy and I still seem OK!

I was on time for a change so not driving hell for leather.

As I thought Jason Lester is still on holiday and was right so I saw his Registrar Rachel Micalles – I am pleased to say the MRI scan has for certain shown up no signs of mets. in the spine anywhere nor in the sacrum or seemingly the pelvis so that is the good news!

The not so good news is that I do have fairly notable degenerative disease in the spine – I offered the explanation this might be as a result of a degenerate life but seems not!

They are not sure what causes it but in the main, in my case, put it down to being the result of wear and tear damage – well I guess I’ve earned that diagnosis!

I gather that being more specific about diagnosis is not really worth the effort at this stage as the likelihood it will be some form of arthritic condition seems certain and clearly I am shrinking!

I have in fact lost about 3 inches in height largely as a result of the disease, though scoliosis and being cut in half do little to help!

SO BACK TO CANCER!:

It seems I have survived remarkably well to date! I am told that the Chemo. did indeed shrink the tumour and seems to have killed off the cancer in the lymph system – SEEMS is an important word here as it is, at this stage hard to tell!

Then having had, as described in this blog, very little side effects from the belting with chemo. it was on to Radio Therapy and 20 rides of around 57 seconds on the Linear Accelerator being cooked with accelerated electrons!

To date I have also survived this well – I was reliably informed that I would most likely be far too unwell to drive myself to treatment every day but fortunately I drove myself on every occasion and apart from around an hour in the evening when I felt very tired I have had virtually no side effects of consequence.

At the moment I am told there is little point in scans or a ‘flexi’ as the full effects have not completed adequately to conclusively show up and apparently the inside of the bladder is likely to be fragile and inflamed AND very prone to the introduction of infection so leave well alone.

Next appointment is now Mid October – which beats even the original 6 month prognosis at best!

SIDE EFFECTS:

It is not strictly true to say ZERO side effects but for sure they have so far been inconsequential.

At one stage for a day or two I had perifferal vision flashes which were a bit odd and I guess must have been an effect on the optic nerve of the blood supply due to cancer (maybe not!).

Fortunately I have not felt sick at any stage and my appetite has been fairly good – largely due to the efforts of Lee in pandering to it! Also to be fair thanks to Nairn in the early stages for the constant supply of relevant mouth washes such as ‘Difflam‘ which controlled the problems of mouth tongue and upper throat ulcers, which only really featured in the early days of Chemo.

There have been periods of mild diarrhea but of little consequence and with the hernia to my nephrectomy wound I have got used to this!

Several of the other patients have said their diarrhea has been quite a problem at times – fortunately mine has been only occasional and no problem.

There is no doubt they have been playing merry hell with my bladder! Particularly during Radio Therapy and I am urinating far too often for convenience! Particularly at night where I rarely get more than around 2.1/2 hours of continuous sleep without a visit to the toilet!

To be fair I do consume a fairly high level of fluid with around 2 pints of fruit juice a day minimum and at least 4 pints of milky strong coffee.

I also consume not less than a pint of yoghurt a day in one form or another – I FEEL that since the system is being bombarded and the gut flora seriously damaged or at best irritated it is worth helping with a replacement together with a consequential amount of fruit relative to normal.

Well if the treatment is working and these are the worst symptoms so far I would call this a very good trade off – though we will not know just how successful the treatment has been for a few months. That said the very fact that I am still here is very much down to the treatment as in April I well recal Jason Lester’s prognosis that without treatment I had a probable 3 months at best 6. Well I’m still here and feeling OK – of course not 100% energy wise but beyond that I can hardly complain and this is after all 13 years after I first presented with ‘frank haematuria’ (I miss spelled that and MS correction was ‘crematoria’ Ha Ha!) as the first symptoms of all the fun to come since!!

Haematuria:

A passing tip for those who are too scared to go to their Doctor if they think they may have blood in the urine the obvious diagnosis is ‘stupidity’!

Blood in the urine is a SYMPTOM only go see your Doctor as soon as possible – most causes can be dealt with simply the others WILL KILL YOU if you let them get hold undiagnosed!

A simple test if you are in doubt is to pass ALL urine into a glass (clean) coffee jar and leave it to stand until naturally cooled blood in the urine is likely to settle out as a distinct band but just because it doesn’t does NOT mean you should avoid seeing your Doctor.

Blood in the urine is also an early sign of possible diabetes and minded that The EU is threatening to withdraw driving licences from those with diabetes who let it reach the stage of injectable Insulin perhaps an early check is well advised!

I’ll try and write up yesterday and today when I get back but yesterday I stood no chance of coherence after a trash night sleep wondering if the alarm would wake me! Then up at 07:00hrs. and uncertain what rush hour traffic would be like.

Hi,

I was quite glad we had arrived early – we had expected rush hour traffic but it seems few people work in South Wales! There were no appreciable build ups of traffic either as we drove past Newport on The M4 nor as we drove on to The Bryn Glas Tunnel and on past Cardiff at around the speed limit almost all of the way – unimpeded by people on their way to work!

Then on to exit 32 and into WhitChurch and Velindre.

The procedure once checked in was to wait in the waiting room of the Machine one is calibrated to! In my case LA3.

The Linear Accelerator is a large piece of equipment with a large price tag of between £1.2 & 1.5 Million + the cost of installation maintenance and a hands on crew of 3 in a spacious and well lit room.

As you wait in the waiting room it is clear many of the patients are regulars and know eachother as they sit there with their issue blue plastic bags as slippers! This is designed, I understand, to track the minimum of outside contaminant into the machine area.

Also it soon becomes clear that dependent on what exact location is being zapped whether you are a drinker or a pee-er! I gather people with prostate cancer being zapped have to drink a certain amount of water before their treatment whereas I with a tumour on the uretal stub on the bladder am expected to empty my bladder as much as I can.

These different perfomances to order are designed to ensure that each treatment the organ concerned will be the same size and position!

The machine or should I more kindly call it an instrument is similar to this one but different!! Though it is made by Elektra:

The gantry itself rotated as do the individual heads as the bed liftf and lowers and moved in the lateral plane to get positioning spot on.

Then it is onto the metal monster (well plastic coated) where one is positioned EXACTLY and the three tattoos from assesment last week are lined up with the lasers  (one on each hip approx on the joint and the third on the pubic bone). Positioning is and needs to be very precise and then a scan is done to identify excatly where the tumour is.
The large Accelerator head is then swung through various positions to take exact measurements befor the 3 operators withdraw to safety and out of the room whilst you are left on the table in exactly the right position for the accelerated particles to be fired through me.

In my case my oncologist has decided the dosage (length of bombardment) and direction, which is 3 positions of the beam/particle accelerator one from aboveslightly obliquely and two from the left side at different angles.

I presume the oblique is to avoid the particles passing through the spine on the way out!

The beam of accelerated particles continues from each position for between 15 & 28 seconds it seems – thereby determining the dose or for that matter damage done!

The Particles enter the body and pass out the other side, I gather doing damage all the way through, so on any one visit a straight line of damage is done through 3 lines but each passes through the tumour. Next visit the beam will pass through 3 slightly different lines but every time Zapping the tumour.

There is no pain sensation in fact the only giveaway the machine is on is that everyone legs it out of the room and one has a David Howarth moment; I hope you have read the WWII epic of survival ‘We Die Alone‘ which he wrote or that comment will make little sense!

Also as the ray actually fires the machine makes a noise similar to an electrical shorting of a cable.

The whole procedure takes only a few minutes and there is no realisation that it has been done!

Then it is clamber off the machine, in my case due to having lost all my stomach muscles and nerve instructions in 2001 due to the operation, subsequent MRSA infection of the wound and gross hernia of the wound – the staff help me back to a sitting position.

Then cheery chats and ‘see you tomorrows’ and back onto The M4 for the 70+ mile round trip. Remembering to take ones blue plastic bag slippers with one!

Radiotherapy saves millions from cancer but can inflict cruel damage, now, at last, there may be an answer…

Perched on a wooden bench inside a metal tank, surrounded by people shrouded in full-face hoods with oxygen pipes, Michael Charter could have been forgiven for thinking he had stepped on to the set of a sci-fi film. 

In fact, the former publisher and businessman was in a decompression chamber receiving huge bursts of oxygen in an atmosphere twice the normal pressure – equivalent to being 40ft under the sea. 

The aim of the treatment, known as hyperbaric oxygen therapy (HBO), was to heal the internal damage caused to his bladder and bowel when he received radiotherapy for prostate cancer 17 years earlier.

‘When my urologist suggested the therapy, I thought he was mad,’ laughs Michael, 70, who lives in Devon. ‘I’d never heard of it. But, as most people with significant and long- term radiotherapy damage will tell you, you will try anything to ease your symptoms.’

While radiotherapy is an effective cancer treatment – experts say it increases survival rates by about 50 per cent – it can damage healthy tissue around the tumour site.

Most vulnerable are the 12,000 patients a year who have pelvic irradiation for bowel, cervical and womb cancer, and those who are treated for head and neck cancers.

‘We think that about 50 per cent of patients who undergo pelvic irradiation will suffer some radiotherapy damage,’ says Dr Jason Lester, an oncologist at Velindre Cancer Centre, Cardiff.

‘Some will simply suffer a change in bowel habits, but others may have distressing symptoms such as bowel ulcers, rectal bleeding, pelvic pain and a lack of bladder control. Some will need a blood transfusion, or even end up with a colostomy bag.’

Dr Lester is one of a growing number of cancer specialists referring these patients for oxygen treatment.

HBO was first used in the Thirties to treat deep-sea divers suffering from decompression sickness and trapped air bubbles (the bends) but quickly it was noted that the treatment aided wound healing.

Dr Phil Bryson, director of the Diving Diseases Research Centre in Plymouth, where Michael had his therapy, says: ‘The principle behind HBO is very logical.

‘We know the protein which controls cell growth and healing depends on oxygen. HBO delivers a huge amount in a short time, and research shows it stimulates this protein to work more efficiently.

‘This is particularly relevant when you consider that radiotherapy damage is characterised by the death of the blood capillaries, which feed oxygen to the damaged area.’

As these capillaries die, the cells get less blood, hence less oxygen. The cells die and a wound forms. Then the cells around the edges of the wound become damaged by the inflammation and they, too, die, causing the wound to enlarge. ‘It’s a vicious circle and there is little medical treatment.

However, in some cases, HBO can halt or reverse that process,’ says Dr Bryson.

Michael Charter was one of those to benefit. After being diagnosed with prostate cancer, aged 53, he had a biopsy and his prostate was removed in 1993. 

Radiotherapy

‘The cancer had spread slightly outside the prostate,’ he says. ‘A biopsy showed that six per cent of the surrounding tissue contained cancer cells, but that was enough for my Harley Street specialist to order a huge dose of radiotherapy.’

Then, 15 years later, Michael began to suffer symptoms.

‘My first symptoms were bladder leakage,’ he says. ‘Then the small leaks became large leaks, and suddenly I was wetting myself. I started using pads. I’d put one on in the morning and it might be full by lunchtime. There were accidents. It was clear something was badly awry.’

Michael’s GP referred him to a urologist who said his situation was probably a side-effect of the surgery – and that little could be done.

‘It was depressing,’ says Michael, who is married to Ingka. ‘From being sociable and outgoing, suddenly I was wetting myself, often in public. It was a terrible blow to my pride and self-esteem.’

Deterioration was rapid. 
Within weeks of the first symptoms, Michael had passed blood in his urine, was suffering from painful bouts of cystitis and had bowel problems.

His urologist suggested he begin using a catheter to empty his bladder. But he still had to sleep on rubber sheets and take changes of clothes with him when he went out.

‘If I’d been consulted, I would have been hesitant about such a large dose of radiotherapy’

All these symptoms are text book radiotherapy damage. Yet no one told Michael exactly why these symptoms had appeared.

‘I thought I was simply living with the effects of having had cancer,’ he says. ‘It wasn’t until I was referred to my current urologist in the autumn of 2008 that I first heard about radiotherapy damage.

‘My urologist told me that today I wouldn’t have been given such a large amount of radiotherapy, and that they might have adopted a watch-and-wait regime.

‘That made me angry. If I’d been consulted, I would have been hesitant about such a large dose.’

Michael’s story is not unique. While the damaging side-effects of radiotherapy have been recognised for decades, until recently patients weren’t told of the risk. Partly this is because of concern that patients may refuse treatment if they knew the risk of damage.

This attitude has left generations of radiotherapy patients struggling to get a diagnosis – let alone treatment – for their problems. However, the bleak truth is that even when the patient is properly diagnosed there is little that clinical or pharmaceutical medicine can do to help.

‘The worst aspect of the condition is that it will probably get worse,’ says Dr Lester.

‘Some doctors have tried rectally administering steroids to calm any inflammation.

‘Others prescribe anti-diarrhoea drugs. But none of these treatments have been properly researched and, in my experience, none of them really work. Some may even make things worse.

‘On the other hand, HBO therapy offers the possibility of improvement. The worst that can happen is that there is no improvement at all.’

But, because relatively low numbers of radiotherapy damaged patients have received the treatment, and no results have yet been collated, there has been no definitive research to show the oxygen therapy can help. It is a situation that many clinicians find frustrating, as it means the treatment is not recognised by NICE.

Patients who wish to use it must apply for funding on an individual basis. Fortunately, this lack of research may be about to change. In a world first, a trial carried out at the Royal Marsden Hospital in conjunction with the Institute of Cancer Research is aiming to establish whether HBO therapy does reverse symptoms.

Michael believes he already has the answer to that question. Initially denied funding by his local health authority, after the intervention of his urologist the money was found for him to attend his first course of treatment in February 2009.

‘I went for 40 treatments over five weeks, and then, this year, I had a top-up of 20 treatments over two-and-a-half-weeks. Each session lasts between two and three hours, which includes three half-hour sessions of breathing pure oxygen through a hood.

‘The chamber often feels warm and your ears pop as it is decompressed. There is always a medically trained attendant and usually up to nine other patients in with me.

‘Each time, the results were astonishing. My bowel problems disappeared completely within days, and although I still have to use a catheter and wear pads, I’m usually dry for a night.

‘To someone who has never had to use pads, it is hard to explain just how wonderful that feels.

‘I am aware that, because the radiotherapy damage is continuous, the benefits of the oxygen therapy will start to wear off after time, but I am due to go for another course next year, which I hope will continue this pattern of improvement.

‘HBO has given me hope that I can reverse some of the worst effects. That is a priceless gift.’

Hi,

Shortly after returning from the canteen our name came up and it was into Clinic with Dr. Lester.

Chatted for a few moments about having eventually got married after 26 years – he was quite miffed as we had beaten him as he had been with his partner for 23 years when they got married! He was originally a Londoner but has married a Welsh girl.

Anyway cutting to the chase:

Firstly he is surprised at how spot on my bloods are considering I’ve got one kidney, one adrenal gland years of cancer and buckets of chemo!!

He sees no problem there and is very pleased he has managed to so successfully trash my bloods every time with chemo thus starving the cancer.

Now for the scan:

Some good and some not so bad!

For the first time he feels he can, since I understand the limitations, talk of a cure.

Obviously this is NOT a cure of cancer but a cure of this outbreak and we then have a watching brief, if lucky, for the next return!

Even if we win this round it could be back in 3 months, or 3 years or if VERY lucky longer.

Caveat:

The tumour on the Uretal stump would seem to have shrunk by about 20% that is OK but not wonderful.

The Radiologist and he had conferred and they believe that the Lymph Node which was necrotic & cancerous seems free of cancer but due to deformation it is hard to be certain.

However it does seem that the Lymph Node on The Femoral Artery which was believed to be cancerous almost definitely isn’t and is, like apparently many lymph nodes, naturally deformed and NOT cancerous.   

So What Now & Why:

OK so the game plan changes!

Dr. Lester is relatively pleased particularly as the cancer HAS responded to chemo on the dosages he advocated but it hasn’t destroyed it totally.

The fear is now that if he ploughs on for the full programme as originally planned the cancer may well become imune to the treatment but the chemo will go on beating me up! I gather the aim is to kill the cancer before you kill the patient!

So we now give it one more round of chemo to ‘drive the nail home’ so to speak.

That is a day 01 which is on Wednesday 08-Jun-2011 which is Carb & Gem

Then the following Wed 15-Jun-2011 which is a day 08 with Gem only.

The next phase will be a schedule of 20 hits of Radio Therapy – 5 days a week for 4 weeks. But more of that later when IO know what the full game plan is.

Lester did say that the sideeffects are less punishing but that I am likely to be very tired during the last 2 weeks – Hardly surprising 80 Mile round trip to Velindre on the same boring bit of M4 every day!!

So that is the game plan so far.

I’m ‘fairly’ laid back about it – but then again what choice do I have! So it is best to not get wound up about it – laughing is always a good medicine and with all my various contacts and blogs I just don’t have time to be concerned! JUST SCARED 😉
